Who we serve
White Glove Behavioral prepares state licenses, accreditation readiness, opioid treatment program approvals and enrollment for behavioral health and addiction treatment programs. Your owner, sponsor or authorized official signs every application and certification, and you pay every government, accreditation and certifier fee directly. Every fee is flat and never depends on admissions, census, revenue or outcome. Clinician licenses and payer contracting are handled by our sister teams.
Facility Licensing
Facility Readiness Review
$149 one-time
Before you spend anything: which state licenses or certifications your services need, the levels of care, the building and staffing rules, and whether a need review applies. Credited in full toward your package when you go ahead.
Outpatient Program License
$1,995 one site, including IOP or PHP
The state license or certification for an outpatient mental health or substance use program, intensive outpatient or partial hospitalization included, for your owner or officer to sign.
Residential or Detox Program License
$2,995 one site
Residential treatment or withdrawal management licensing, with the heavier staffing, building and medical rules planned in.
Additional Site or Service Component
$595 each
Another location, or another service or level of care added to an existing license.
Policy and Procedure Manual
$1,495 add-on
Policies written to your state's rule, section by section, so the inspector finds each requirement where they expect it.
Ownership Change or Relocation
$795 per change
The state notices or new license an ownership change or move triggers, with Medicare and Medicaid updates planned in.

Accreditation Readiness

A gap analysis, policy work and coaching before a CARF, Joint Commission or COA survey. You apply and are surveyed; we prepare you.
Accreditation Readiness, Each Added Program

Mock Survey
$1,495 per survey
A full rehearsal of an unannounced survey, with a written findings list. Charts are reviewed on site; we never take them.
ASAM Level of Care Certification Readiness
$1,995 per program
Readiness for CARF's ASAM Level of Care certification or a state's ASAM designation, level by level.
Opioid Treatment Programs
Opioid Treatment Program Approval Package
$6,995 plus the state license package
The four approvals in order: the state license and opioid treatment authority, SAMHSA certification (SMA-162), OTP accreditation and DEA registration. The sponsor signs and submits each one from the sponsor's own accounts.
OTP Medication Unit
$1,495 per unit
A medication unit added to a certified program, with the SAMHSA, DEA and state steps it needs.
OTP SAMHSA Certification Renewal
$595 per renewal
The SAMHSA certification renewal prepared for the sponsor to sign and submit from the OTP Extranet.
DEA NTP Registration Renewal
$249 per renewal
The narcotic treatment program registration renewal information, prepared for your officer to sign and pay. We never hold a DEA power of attorney.
Enrollment and Ongoing Compliance
Medicare CMHC Enrollment
$1,995 per CMHC
The 855A for a community mental health center, with the 40 percent rule documentation organized for your own independent certifier. Your authorized official signs; we submit as your PECOS surrogate.
Medicaid Facility Enrollment
$499 per state
State Medicaid enrollment for the program, with its license and screening rules planned in.
Recovery Residence Certification
$795 first house
House policies and inspection readiness for NARR-affiliate or state certification. The operator applies; we prepare.
Recovery Residence Certification, Each Added House
$295 each added house
Another house under the same operator's certification.
CCBHC Readiness Gap Assessment
$2,495 one-time
Where your clinic stands against the CCBHC criteria, in writing. The state certifies; we never file for you.

Renewal Tracking
$299 per license, certificate or registration, per year
State licenses, SAMHSA, DEA, accreditation and Medicaid dates on one calendar, each prepared ahead of its date.

The prices listed above are our service fees only. Costs paid directly to third parties are set by others and change from time to time, so we confirm them at intake:
- State license and inspection fees: set by each state, often per site or per bed, plus any fire or building inspection.
- Accreditation fees: set by CARF, The Joint Commission or COA for your survey.
- DEA registration fees: paid to DEA with each narcotic treatment program registration and renewal.
- The CMS application fee: $750 for 2026, paid to CMS with a CMHC enrollment or revalidation.
- Recovery residence certification fees: set by the NARR affiliate or state that certifies the house.
