SAMHSA certifies an opioid treatment program for up to three years and expects renewal on Form SMA-162 in the final year. If the accrediting body grants only one year, SAMHSA may grant a one-year conditional certification, and the program must reach standard accreditation within it. An expired certification sends the program back to provisional certification as a new applicant.
An opioid treatment program's (OTP's) SAMHSA certification is not permanent. "Certification shall be granted for a term not to exceed 3 years," and renewal happens "during the final certification year" (42 CFR 8.11(a)(3)). The term an OTP actually receives depends on its accreditation, so renewal planning starts with the accrediting body's calendar, not SAMHSA's.
How long does certification last?
Up to three years. SAMHSA's "Become an OTP" page says that once certified, "OTPs must renew certification annually or every three years depending on the accreditation timeframe awarded." The link is in Part 8 itself. An accrediting body grants three years when an OTP meets the federal standards with only minor conditions to correct, and one year when significant conditions must be corrected (42 CFR 8.4(b)(3)-(4)).
What is conditional certification?
An OTP seeking renewal that "ha[s] been granted accreditation for 1 year by an Accreditation Body ... may receive a conditional certification for one year unless the Secretary determines that such conditional certification would adversely affect patient health" (8.11(a)(5)). The OTP must then obtain a standard three-year certification within that year. If it does not get standard accreditation in time, "the OTP's conditional certification will lapse, and the Attorney General will be notified that the OTP's registration should be revoked." In other words, a failed conditional year can reach the DEA registration.
The accrediting body's side of this: when it verifies that the OTP corrected its significant conditions on schedule, "it shall extend the OTP's accreditation period by an additional two years" (8.4(b)(4)). That extension is what lets the program move from conditional to standard certification.
When should the renewal be filed?
In the final year of the certification period. Part 8 says OTPs "are expected to apply for certification renewal during the final year" and "should take steps to ensure that administrative tasks associated with renewal are completed before the OTP's certification expires" (8.11(a)(4)). The renewal goes in on Form SMA-162 through the OTP Extranet, signed and submitted by the program sponsor.
Two practical rules from SAMHSA's FAQ:
- Send the survey report. A renewal shows "in processing" until it is marked complete, and "the OTP must submit a copy of the full accreditation survey report" to complete it. SAMHSA asks programs not to file a duplicate SMA-162 while it waits, because that "will complicate the OTP's renewal application process."
- Ask for an extension before expiration. If a delay is coming, email the assigned compliance officer "prior to the certification expiration date" with the reason. If the delay is an upcoming accreditation survey, include documentation from the accrediting body, and still submit the renewal SMA-162. Part 8 provides for the same: an extension "may be requested by submitting to the Secretary a statement justifying the extension" (8.11(a)(4)).
What happens if certification expires?
The program starts over. "OTPs whose certification has expired, and who seek re-certification, will be considered 'new' programs and will be required to apply for provisional certification" (8.11(a)(6)). The re-certification application must "include an explanation of why the OTP's most recent certification expired and information regarding the schedule for an accreditation survey" (8.11(b)(8)). That also means the provisional-certification path, with its need for state and DEA approvals, applies again. See provisional certification.
Can certification end early?
Yes. If an accrediting body revokes an OTP's accreditation, SAMHSA may investigate and move to revoke certification, or set a corrective plan that lets certification continue while the program seeks reaccreditation (42 CFR 8.13(a)). SAMHSA may also revoke certification after notice and a hearing for misrepresentation, failure to meet the standards, failure to provide requested records, or refusal of a reasonable inspection request, and it may suspend immediately when public health or safety requires (8.14). A suspension leads SAMHSA to notify DEA that the registration should be suspended (8.14(d)).
Which dates belong on the renewal calendar?
- The SAMHSA certification expiration date and the start of the final year.
- The accreditation expiration date and the accrediting body's survey window, which drives everything else.
- Any correction deadline from the last survey; the accrediting body allows up to 180 days (8.4(b)(1)).
- The annual DEA renewal, which runs on its own one-year clock; see DEA NTP renewal.
- The state OTP license renewal date.
Our renewal calendar post puts these next to the program's other licenses.
How do we help with renewal?
Our OTP SAMHSA Certification Renewal service is $595: we prepare the SMA-162 renewal information, assemble the accreditation documents SAMHSA needs to mark it complete, and draft any extension request for the compliance officer. The program sponsor signs and submits from the sponsor's own Extranet account. A Mock Survey ($1,495) before the accreditation survey is the best protection against a one-year result, and Renewal Tracking ($299 per credential per year) keeps every date in view. See pricing.
Common questions
- How often does an OTP renew SAMHSA certification?
- Up to every three years, in the final year of the term. SAMHSA says renewal is annual or every three years depending on the accreditation timeframe awarded.
- Why is my SAMHSA renewal still in processing?
- SAMHSA's FAQ says the renewal stays in processing until the OTP submits a copy of the full accreditation survey report. Do not file a duplicate SMA-162 while it waits.
- What happens if OTP certification expires?
- The program is treated as new and must apply for provisional certification, explaining why the last certification expired and giving its accreditation survey schedule.
